Waiver, Assumption of risk and agreement to hold harmless
This is a Contract between WHITE MOUNTAIN K-9 ACADEMY LLC(hereinafter called "academy") and the pet owner/student whose signature appears below.
1) I agree to pay the rate for training, in effect on the date pet or student is registered into the training program (as indicated above). It is understood that refunds will not be issued for any reason after the first class.
2) By signing this contract and registering the pet with academy, I certify that rabies, DHLP, Canine Cough, and Parvo vaccinations are current.
3) I specifically represent to academy that my pet has not been exposed to rabies or distemper within a thirty day period prior to beginning training.
4) I further agrees to be solely responsible, financially or otherwise, for any and all acts or behavior of said pet while it is on the property or sanctioned property of academy.
5) I hereby waive and release academy, it's employees, owners and agents from any and all liability of any nature for injury or damage which I or my dog may suffer, including specifically, but without limitation, any injury or damage resulting from the action of any dog, and I expressly assume the risk of any such damage or injury while attending and training sessions or other function of academy, or while on the training grounds or the surrounding area thereto.
6) It is understood that the cost for this course is $800, including the necessary tools and supplies. There is an additional charge of $80 for books. The book charge is optional and you may choose to purchase your books from a source other than us, If you choose to locate your own books, this MUST be stated prior to payment and admission to the course. 50% of tuition is due 30 days prior to class start with the remainder due at the start of class. Your 50% payment is not refundable if you decide to cancel prior to the start of class. After the first class session, there will be no refunds issued for any reason. |
